However, recent data has revealed a disturbing trend – a significant increase in deaths among U.S. residents who traveled to the D.R. for these procedures. According to a study published in the Morbidity and Mortality Weekly Report (MMWR), there were 93 deaths recorded between 2009 and 2022. Although there was an average of 4.1 deaths per year during the period from 2009 to 2018, that number rose to 13 deaths in 2019-2020, with a peak of 17 deaths in 2020.

One particular procedure, known as gluteal fat transfer or the Brazilian butt lift (BBL), was found to be closely linked to the majority of deaths. In fact, within the subset of deaths occurring during peak years, a staggering 92% were tied to gluteal fat transfer. Medical records and autopsy reports indicated that embolic events, such as fat emboli or venous thromboembolism, were responsible for the fatalities. Risk factors such as obesity and the performance of multiple procedures during the same operation contributed to the increased likelihood of these tragic outcomes.

The Dominican Republic has become an attractive hub for medical tourism due to its lower costs, shorter wait times, and its proximity to the United States. Many individuals seek out cosmetic surgery in this country, hoping to achieve their desired aesthetic results at a fraction of the cost they would incur in their home countries. However, the rise in deaths is a stark reminder that the pursuit of beauty should never come at the expense of safety and well-being.

The MMWR study highlighted the potential for preventing or mitigating risks to patients through enhanced surgical protocols and postoperative care. Proper prophylactic measures against venous thromboembolism, as well as stricter adherence to infection control and prevention practices, could contribute to safer outcomes. The authors emphasized the importance of comprehensive training for surgeons and staff, ensuring that they are equipped with the knowledge and skills to minimize the potential risks associated with these procedures.

One notable limitation of the study was the lack of reliable statistics regarding the total number of cosmetic procedures performed on U.S. citizens in the Dominican Republic each year. This information gap hinders a comprehensive understanding of the scale and scope of the problem. Furthermore, another MMWR report identified 15 cases of Mycobacterium abscessus infections among patients who underwent cosmetic surgery at a single facility in south Florida. The majority of these cases involved individuals who had received gluteal fat transfer, further highlighting the risks associated with this particular procedure.

The investigation into the cluster of infections in Florida revealed multiple lapses in infection control and prevention protocols at the implicated clinics. Gaps in environmental cleaning practices, improper use of personal protective equipment, and inadequate disinfection during the reprocessing of surgical devices were identified. These findings underscore the crucial need for rigorous adherence to strict infection control measures in all healthcare settings, particularly those that perform cosmetic procedures.

Notably, South Florida has seen some of the highest numbers of deaths associated with gluteal fat transfer in recent years. The region has also experienced an increase in the proliferation of “budget” clinics that prioritize profit margins over patient safety. In response, the local authorities have started to crack down on these establishments in an effort to protect patients from undue harm.

The rise in deaths among individuals seeking cosmetic surgery in the Dominican Republic is a cause for serious concern. It highlights the urgent need for improved surgical protocols, comprehensive training for healthcare professionals, and strict adherence to infection control measures. Furthermore, it serves as a reminder that individuals should prioritize safety and choose reputable healthcare providers over cost considerations when undergoing cosmetic procedures.
